First Visit

Your initial appointment usually will consist of an oral cancer exam, periodontal exam, necessary x-rays, soft and hard tissue exam and a review of your medical history. If you have received regular cleanings and are periodontally healthy, a cleaning will be performed at the first visit.  However, if you have periodontal problems (gum disease), or a complex treatment plan that has numerous options to discuss,  treatment will not start until your second appointment.

Please assist us by providing the following information at the time of your consultation:

  • Request any x-rays taken by a previous dentist, if applicable, prior to your appointment.
  • Fill out Online Registration prior to your first appointment.
  • Bring a list of all medications you are presently taking.
  • If you have dental insurance, bring a copy of your insurance card.
  • Please bring a valid Identification Card.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility.

Our office uses a digital panorex and digital intra-oral x-rays to keep your radiographic exposure to a minimum, along with being able to diagnosis oral conditions in the most efficient manner.
